A vallecular cyst is a benign laryngeal that rarely causes symptoms in adults. We report rare case of an infected man who his 60s with diabetes mellitus. The patient presented to us acute onset voice muffling and respiratory distress. He showed signs sepsis upper airway obstruction. had spikes high-grade fever elevated total white cell count (predominantly neutrophil count) C reactive protein level. Plain cervical X-ray revealed large, smooth, lobulated mass the area. Direct laryngoscopic findings supported diagnosis suppuration, which originated from vallecula fossa. was drained excised completely, pus culture presence Klebsiella pneumoniae. responded well administration intravenous cefuroxime for 2 weeks according sensitivity test. discharged after completion course antibiotics, having recovered fully. This illustrates versatility plain giving clues about this rare, unexpected pathology.
